Mar. 1, 2013 — Drug companies spent nearly $84 million marketing pharmaceuticals in the District of Columbia in 2011, including an outlay of nearly $19 million for gifts given to physicians, hospitals and other health care providers, according to a report by researchers at the George Washington University School of Public Health and Health Services (SPHHS). The report notes that 12 physicians in the District received gifts (including consulting payments) that totaled more than $100,000 apiece that one year alone.
"There is nothing inherently wrong with such gifts," said Susan F. Wood, PhD, lead researcher and an associate professor of health policy and of environmental and occupational health at SPHHS. "However, this report draws attention to the amount being spent on marketing drugs and raises questions about whether some heavily marketed drugs may be prescribed more extensively than is appropriate."

The report fulfills the requirement of a 2004 law in the District of Columbia that requires all pharmaceutical companies to file annual reports describing their prescription-drug marketing activities in the District. The AccessRx Act also requires analysis of these reports to determine how pharmaceutical marketing may affect healthcare services in the District.

In fact, a previous study by Wood and her colleagues showed that drug companies making antipsychotic drugs gave a disproportionate amount of gifts and payments to District psychiatrists who treat Medicaid patients. Close ties between the drug companies and psychiatrists might have led to inappropriate prescribing for Medicaid patients, and particularly for children, according to D.C. Council Member David Catania, who held a hearing on the issue last November.

"Antipsychotics are often prescribed to children who are not in fact psychotic but have behavioral problems," said Adriane Fugh-Berman, MD, a co-author on both reports and an associate professor of pharmacology and physiology at Georgetown University Medical Center. "These powerful drugs also act as sedatives and might lead to less disruptive behavior, but at what cost?" Antipsychotics are also associated with health problems including obesity and Type II diabetes, she said.

The current report doesn't single out specific drugs or name any doctors or other providers but it does give an overview of the money that drug companies spent on marketing their products in the District. The report notes that drug companies spent a total of $83.7 million for advertising, gifts and aggregate expenses in 2011 -- slightly less than 2010. Still, the report found that a small number of companies -- 23 out of 158 -- reported marketing expenditures of more than $1 million apiece. "That is an astonishing amount of money being spent on marketing prescription drugs in the District," said Wood.

The report also found that 22 percent of total marketing expenditures, or $18.9 million, went to "gifts" -- a category that included grants, speaker's fees and food. Drug companies may treat a select group of doctors to dinner and a sales pitch at a local restaurant, as well as provide delivered meals to medical practices, Wood said.

Hospitals, clinics and other organizations received gifts totaling $9.7 million, and individuals received $9.2 million. Some nurses and pharmacists received gifts in 2011 but most of the pharmaceutical company largesse, nearly 82 percent went to doctors. Most gifts to doctors were described by the drug companies as speaking fees and were paid in the form of cash or checks, the report said.

Other findings of the report include:

  • Out of nearly 3,400 physicians in the District who received at least one food gift, 444 received ten or more meals from pharmaceutical companies during 2011 and 33 physicians got 52 or more of these food gifts. This finding suggests some physicians are dining with drug reps on a weekly basis.
  • The top ten professional organizations (representing health professionals in a specific specialty or demographic group) received a total of $3.5 million, with half of those gifts valued at $20,000 or more.
  • The top ten Disease-Specific Organizations based in the District received $2.1 million in gifts such as cash or checks from drug companies. These organizations often represent and advise patients, and the concern is that large gifts could sway such groups to favor or recommend the company's products, the report said.
The report, which was commissioned by the Department of Health (DOH) in the District of Columbia, notes that under the Affordable Care Act pharmaceutical companies will begin publicly reporting gifts to physicians and teaching hospitals in September 2014. At that time, the authors say that the District could publicly release the names of all gift recipients, a step that would allow patients to have information about the financial relationship between drug companies and all of their health care providers.

An outspoken state assemblyman who serves on the chamber's Alcoholism and Drug Abuse Committee — and voted against medical marijuana — has been busted for possessing pot, state police said Friday.
Assemblyman Steve Katz, 59, a conservative Republican from the Hudson Valley, was pulled over on the state Thruway for going 80 miles an hour  in a 65 mph zone when a trooper detected a “slight odor” of marijuana, state police spokesman Sgt. Don Baker said.
The trooper asked Katz if there was any pot in the car - and the assemblyman said “yes” and handed over a small bag that contained less than 25 grams, Baker said.
The trooper did not see evidence that Katz was driving while impaired so he ticketed him for unlawful possession of marijuana, a noncriminal violation, and speeding — and let him drive off, Baker said. Katz was heading to Albany at the time.
“If there was a concern that he was smoking while driving, we would have brought in a drug recognition expert to determine if he was under the influence, but there was no indication that he was,” Baker said.
Katz — due to appear in Coeymans Town Court on March 28 — declared in a written statement Friday afternoon that he would not let the "unfortunate incident ... impede my public service.”
“This should not overshadow the work I have done over the years for the public and my constituency,” he said. “I am confident that once the facts are presented that this will quickly be put to rest."

As as assemblyman, Katz last year voted against a bill to legalize medicinal marijuana.
In a mailer to his constituents, he talked about how “our community has been stricken with an increase in drug use and drunk driving by our youngest citizens.”
And on March 8, he put out a statement decrying his community’s “struggle against illegal drug culture and the abuse of narcotics.”
Katz has been outspoken about the bad-boy culture of Albany. In January, he criticized Assembly Speaker Sheldon Silver for his handling of the sexual harassment complaints against Assemblyman Vito Lopez (D-Brooklyn).
Katz — a vetererinarian, from Putnam County — has had his own controversies. He has been arrested two times on charges related to his veterinarian practice, including the illegal dumping of the body of a dead dog. He was never convicted.
Brooklyn Sen. Martin Golden, a fellow Republican, called it “hypocritical” for Katz to have voted against pot legislation and then get charged with possession.
“Be true to yourself,” said Golden, who sees pot as a gateway drug. “If you like marijuana and smoke marijuana, at least vote for it.”

. – I have never tried anything related to drugs myself, but still what is told here about marijuana is so dramatic that I could not resist publishing it. I could choose to make a long article about this or I could let the article by Steve Elliott, linked above speak for itself. Even though I have been against drugs I decided to let Elliott share his knowledge, even here on State of Globe. The reason is simple. My whole life I have spoken out against drugs, but I have at the same time read article upon article and seen videos about the alleged healing powers of marijuana in particular. At the same time I have seen that the pharmaceutical industry has a long story of fighting alternatives to their patented and usually very costly solutions. A year or so ago I also saw that chemotherapy used against cancer is found to spread the cancer, even though it may kill the main tumor. The whole concept of fighting cancer has a growing credibility problem.

Hartford - A combination of Percocet, Oxycontin, Vicodin and a number of other drugs made Robert Specht of Hamden a "puddle" in bed for five years. He had been prescribed the narcotics to help recover from a foot surgery.
But it wasn't until he got off the narcotics and began using cannabis, or marijuana, that he could once again function.
On Monday, Specht told members of the General Assembly's Finance, Revenue and Bonding Committee that he believes that pain should be added to the list of qualifying conditions for the state's medical marijuana program.
He was testifying in support of Senate Bill 1117, which proposes to include "pain that is treated by a pain management specialist" as one of the debilitating medical conditions that can be treated with medical marijuana. The bill also proposes taxing producers $250 for each pound of marijuana delivered to a dispensary.
Both supporters and opponents of the bill made passionate arguments about whether to include "pain" as a debilitating medical condition that deserved medical marijuana.

Marijuana is a PTSD nightmare cure. At least in my case, and among a group of people I know who suffer from PTSD nightmares.
I have searched quite a bit, and as far as I can discern, the fact that marijuana can stop PTSD nightmares in their tracks is NOT generally known. This really must be corrected because there are, no doubt, millions of people out there, police, fire, military, and just the general population, who wake up every night from recurring nightmares, and they don’t know that marijuana would stop these traumatic dreams completely.
I have been trying to think of a way to get the word out to the public on the use of marijuana to stop PTSD nightmares, and this looks like a good place to try.
My story: I am a Vietnam veteran who served in Vietnam in 1968/1969. After I had been there almost a year, I was involved in an attack by the Viet Cong on the Marine Combat Base at Phu Bai, South Vietnam. It is not necessary to go into detail, just know that I thought I was about to be killed and went through all the trauma that this kind of thing can do to a person.
I obviously survived, and went on about my business, and then about two or three weeks after the incident, I started reliving this traumatic event every night in horrible nightmares. I didn’t think too much about it at the time. I thought this kind of thing was probably normal for someone who experiences such shocks to the mind. But as time went on, the constant nightmares really became debilitating.
Then, I was introduced to Vietnamese marijuana. And the nightmares quit. I still did not think much about it, thinking the nightmares had probably run their course, and did not connect the stopping of my nightmares with smoking marijuana.
I smoked marijuana steadily for a few years after that, and then decided to quit because I was trying to get a job that required a drug test. After I quit smoking, within just a week maybe two, my PTSD nightmares were back full-blown! It was like they never had stopped. It was really quite disturbing to realize that something inside me was causing me this kind of distress even these many years later.
I still did not connect the marijuana with having anything to do with my nightmares.

Puts what in perspective? That pro football players are bigger and faster than ever? That when they collide, the results are frightening? Here’s some perspective: The kinetic energy generated by athletes of this size and speed has out-distanced the ability of the human body to withstand traumatic injury.
 
Certainly, it’s nothing that the National Football League is unaware of. Last June, more than 2,000 former players filed a lawsuit that could potentially cost the league billions. The suit claims that the NFL hid information that linked football-related head trauma to permanent brain injuries. Thousands of former players state that the head trauma they sustained as players has diminished their quality of life, with effects including (but not limited to) chronic physical pain, depression, dementia and suicide.

The players and their families claim that the serious health ramifications of continual blows to the head, which are integral to the sport, were hidden and that the NFL “generated false findings” to that end. The lawsuit also includes helmet maker Riddel. The players are seeking unspecified compensation for the care and treatment of neurological conditions associated with brain trauma.
The players claim that they expected injuries as a part of playing the game, but what they didn’t expect was that they would prematurely descend into the kind of dementia and mental deterioration that usually strikes much older people. The gravity of the situation was brought to public attention by the recent suicides of three former NFL players who were suffering from ailments similar to those associated with Alzheimer’s or other neurological diseases. In February of last year, All-Pro defensive back Dave Duerson shot himself in the chest and requested in his suicide note that his brain be studied for damage.
Dr. Ann McKee, a neurologist studying brain trauma in Bedford, Massachusetts examined Duerson’s brain and determined that he “had classic pathology of Chronic Traumatic Encephalopathy (CTE) and no evidence of any other disease.” McKee found that the repeated blows to the head experienced by football players accelerate mental decline by creating a protein that accumulates in brain cells, disabling and eventually destroying them. The symptoms of CTE resemble those of Alzheimer’s disease—personality changes, irritability and irrational behavior that progress to dementia. Bedford says that she has seen some level of CTE in every football player she has examined. Former NFL players report suffering from dementia, Alzheimer’s or other memory-impairment diseases at a rate five times higher than that of the national average for their age group.
With all the attention now focused on the terrible toll that professional football takes on the human brain, a curious and perhaps provocative question needs to be posed to NFL administrators and officials: Could marijuana help?
At first glance the question seems flippant and irrelevant, but when one looks at the evidence from a growing body of science involving the study of plant cannabinoids—the compounds found only in cannabis—the idea of using marijuana to treat and possibly repair the injured brains of athletes seems plausible. Researchers from around the world have found that THC, CBD and other cannabinoids have remarkable neuroprotective properties and also trigger the brains repair mechanism to generate new cells.
Research published as far back as 1998 found that “Cannabidiol (CBD) and THC were shown to prevent hydroperoxide-induced oxidative damage as well as or better than other antioxidants…” and that CBD was more effective for protecting the brain from damage resulting from injury than the antioxidants Vitamin C or E. Dr. Raphael Mechoulam, the Israeli researcher who first identified THC as the euphoric agent in marijuana said that “the endocannabinoids anandamide and 2-AG, as well as some plant and synthetic cannabinoids have neuroprotective effects following brain injury.” Another study, carried out at Michigan State University found that after 48 hours of treatment, THC significantly reduced tau deposits in the brain.
One of the plaintiffs in the NFL suit is Kevin Turner, 42, a former Philadelphia Eagles running back who is stricken with ALS, better known as Lou Gehrig’s disease, which he blames on repeated head blows.
